Someone used this expression about an escort (Aleks Buldocek) in The Deli. I've always wondered- is this expression used for both bottoms and tops. I always thought it was more appropriate to be used for bottoms. But as far as I know, Aleks is a strict top.

Same here. It's the equivalent to "great lay" or "good in bed." It is not used to denote who's the top and who's the bottom. If it were, no one would say "she fucked him" (where both are cisgender and she didn't use a strap-on or dildo). But that usage is not uncommon.

 

As poolboy mentions, ditto for sex between two cisgender women.
